Funded through a Ministry of Health (MOH) initiative, ChronicCare helps 600+ doctors across Ontario support patients with diabetes, high cholesterol, and other eligible conditions. DoctorCare has helped more than 42,000 patients through the ChronicCare service.
In-clinic sessions lead by experienced health educators
Friendly, in-clinic education sessions are designed to improve your patients’ health, knowledge and quality of life. They are led by experienced DoctorCare educators, who include registered nurses and registered dieticians trained in chronic disease self-management and healthy lifestyle coaching. Health educators are available in 35 different languages.
They help your patients with assessments, goal-setting, and recommendations, using current clinical practice guidelines and evidence-based sources from the Heart & Stroke Foundation of Canada, Diabetes Canada and The Canadian Lung Association.
