You’re likely visiting our website because you received a call or a letter about a program that we are helping your family doctor with. Here’s a little more information about us and how we’re helping your doctor.

Why we contacted you…

As you probably know by now, we’re DoctorCare. One of our team members gave you a call on behalf of your family doctor to let you know about a health education or health prevention program that is being run in their office. If you missed the call or have any questions, give us a call at 647-729-1513 and we’d be happy to discuss the program your family doctor is offering. If you prefer, of course you’re also welcome to discuss this program with your doctor at your next scheduled appointment.

Who we are…

As part of the government of Ontario’s push to better the health of Ontarians, doctors are encouraged to provide programs or services that increase awareness about health and educate people about certain health conditions. DoctorCare’s role is to create efficiencies by removing the administrative tasks of creating and managing these programs and services from doctors (connecting doctors with dieticians and other specialists who will deliver the programs or services through the doctor’s office; contacting patients; etc.), so that doctors are focused on what they do best: providing health care.

The doctors we work with are committed to caring about your personal health information, and so are we. Although we are not the doctor’s direct staff, we are their agents. As agents, we bind ourselves, just as a doctor’s staff would, to maintain confidentiality with all information that we receive in the course of our work for the doctor. FAQ For Patients

Q: What is a Family Health Team?
A: A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you – the patient. It is designed to give doctors support from other complementary professionals, most Family Health Teams will consist of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible.

 

Q: My doctor recently stopped offering me an annual physical exam?
A: An annual physical examination may not be required by many patients, but an annual health review comprised of vital signs measurements and an update of your medical, social and family history is recommended. The new Ontario guidelines for pap testing are every 3 years for those who are sexually active, aged 21-69 years and who have had previously normal pap results. Your healthcare provider may suggest that you come in more often if you have had an abnormal pap in the past.

 

Q: What does a nurse practitioner do?
A: Nurse practitioners are registered nurses who have additional education and nursing experience. Working collaboratively with other health-care providers, they provide quality care for patients, order tests, prescribe medications, and diagnose and manage chronic illnesses.
