In an effort to increase the sustainability of Ontario healthcare, the government introduced a chronic care program in 2013, essentially paying for ancillary providers, like dieticians, to provide ongoing support to patients with chronic diseases such as Diabetes and High Cholesterol. It is a very valuable offer for doctors and patients alike but many doctors do not have the time to manage all the logistics around such program.
